Privacy Acknowledgment

Generic University of Medicine (GenMed will collect personal information on this application form, including some sensitive information such as:

• Your name.
• Your gender.
• Your birth date.
• Your citizenship/nationality, country of residence, and time zone.
• Your email address, phone number, and street address.
• Your profession.
• Your government-issued photo ID.
• Education history, employment history, and references.

In addition Generic University of Medicine may request other documents and follow-up information after reviewing your application.

GenMed collects this personal information in order to process your application and consider your eligibility. If you do not provide your personal information we may not be able to process your application. We collect, use, disclose, and handle your personal information in accordance with the GenMed Privacy Policy.

The GenMed Privacy Policy contains instructions for opting out of marketing communications, accessing and correcting your personal information, and making a privacy complaint.
